Service Description
What to Expect from Your Assessment Your personalised assessment includes three key stages: • Pre-session: You’ll receive a 3-day food diary template and a health and lifestyle questionnaire to complete before we meet. • During the session: We’ll review your dietary intake, discuss how it relates to your current health, and collaboratively set SMART goals tailored to your needs (this will be a a 30-60 minuite zoom call alongside presentation). • Post-session: You’ll receive a PDF care plan outlining your goals, along with personalised resources to support your progress.

Initial Consultation Policy The initial consultation is a complimentary session designed to assess whether Daniel Essom, Registered Associate Nutritionist (AfN), is best placed to support your individual needs through one of the available programs. This conversation will explore your general goals and circumstances, provide an overview of the program options, and help identify which package may be most appropriate for you. The consultation is intended to guide your decision-making and determine if we are the right fit to work together. Please note, this consultation should not replace medical advice from your GP or healthcare provider. It is not a clinical appointment. No nutritional advice or personalised recommendations will be given. The session is purely an informal discussion to understand how I may be able to help you. Key Points: • Duration: Approximately 15–30 minutes • Format: Conducted via phone or video call • No obligation to sign up • No medical history will be taken or analysed • No individualised nutrition advice or treatment will be provided • Any personal information shared will be handled in accordance with ourData Protection Policy If you decide to enrol in a program, a full assessment and personalised consultation will follow as your starting point.
